Abstract

Early Orthognathodontic Correction to Rebalance Chewing and Cognitive Potential During Growth

The complexity of orthognathodontics stems from the correlation of malocclusions with oral function and body posture, making the field of interest not only in dentistry but also in medicine in general. Furthermore, considering that malocclusions develop during a child's psychophysical development, the difficulties of the topic are evident. This characteristic confers considerable responsibility on orthognathodontics, but also considerable opportunities. So, what can we do? Is it possible to transform a complex case into a simple one? And what do we need to know to avoid transforming a simple case into a complex and unstable one? The answer lies in understanding and respecting the physiology of the system. Knowledge is fundamental, and recent research has shown that masticatory function plays a role not only in the balance of the stomatognathic system and central motor control, which we have long known, but also in the development of memory networks and cognitive activity during a child's growth. These unexpected results have been clearly demonstrated by basic and clinical research: chewing foods and the associated activation of the masticatory muscles are a fundamental stimulus for the formation of new developing neurons. The importance of understanding the chewing alterations related to malocclusions is therefore evident, in order to understand their etiopathogenesis and establish logical and coherent diagnoses and treatment plans. The presentation will focus on the scientific findings of the main alterations in masticatory function related to malocclusions (part one) and the reasons why functional appliances (Function generating Bites FGB) in addition to the orthognathodontic correction, are able to restore stable chewing balance (part two with case presentations). Achieving functional balance allows for harmonious dento-alveolar-basal growth, transforming a complex case into a simple alignment case. Knowledge and adherence to evidence-based correction sequences are the key to a successful decision-making strategy. This clear and consistent approach gives the orthodontist the satisfaction of achieving the functional rebalancing that is the basis of aesthetically pleasing and stable dentition over time, and the child the opportunity to fully develop their innate cognitive potential, as demonstrated by research, for improved well-being and quality of life.
